The use of a modified rescue cervical cerclage in a diamniotic dichorionic twin pregnancy that had cervical dilatation

Objective We describe the successful utilization of a modified rescue cerclage in 32-year-old G4 P1213 woman Afro-Caribbean descent with Diamniotic Dichorionic twin gestation who presented at 20 weeks and 2 days for routine anomaly scan was found to have dilated (1.7 cm) short (0.49 cervix. Design A standard prophylactic cervical done 12 weeks' using Mersilene™ tape. For cerclage, client placed steep Trendelenburg position, back filling bladder. The cervix grasped ring forceps, an inflated foley balloon catheter introduced into opening aid reduction membranes. In addition initial two 1- Prolene™ sutures were inserted distally tape second proximally vesicocervical junction (reinforcement cerclages). first provide additional structural support prevent prolapse membrane. used close external or minimize risk ascending infection. procedure uneventful postoperatively she treated antibiotics, vaginal micronized progesterone venous thromboembolism prophylaxis. membranes remained intact. Result twins successfully delivered by scheduled repeat caesarean section 36 Twin 1 Apgar scores 9 5 min, respectively, 8 10, respectively. Conclusion context challenging obstetric history as this case, more aggressive approach inclusive reinforcement may be considered. outcomes case further reiterates need larger clinical trials pregnant individuals dilations entry.
